Sternal Wound Reconstruction With Omental Flap for Poststernotomy Mediastinitis

چکیده
Figure 1. Mediastinal defect with exposed pros-thetic aortic root graft. DESCRIPTION A 70-year-old woman with short stature and hypermastia underwent ascending aortic aneurysm repair with a prosthetic graft, complicated by postoperative mediastinitis and sternal dehiscence. She was treated with aggressive sternal debridement and negative pressure wound therapy for 1 week (Fig 1) prior to definitive closure with an omental transposition flap and eventual skin graft to the sternal wound. Her hospital course prior to sternal wound closure was also complicated by a deep venous thrombosis (DVT) and pulmonary embolus (PE) for which she was treated with anticoagulation.

Sternal reconstruction after post-sternotomy mediastinitis
BACKGROUND Deep sternal wound complications are uncommon after cardiac surgery. They comprise sternal dehiscence, deep sternal wound infections and mediastinitis, which will be treated as varying expressions of a singular pathology for reasons explained in the text. METHODOLOGY AND REVIEW This article reviews the definition, prevalence, risk factors, prevention, diagnosis, microbiology and ma...
متن کاملImmediate Debridement and Reconstruction with a Pectoralis Major Muscle Flap for Poststernotomy Mediastinitis
BACKGROUND Poststernotomy mediastinitis is a rare, but life-threatening complication, thus early diagnosis and proper management is essential for poststernotomy mediastinitis. The main treatment for mediastinitis is aggressive debridement. Several options exist for reconstruction of defects after debridement. The efficacy of immediate debridement and reconstruction with a pectoralis major muscl...
متن کاملLaparoscopic Repair of a Subxiphoid Hernia After Omental Flap Reconstruction
Introduction: Hernias anterior to the sternum have rarely been reported and usually occur in the context of abdominal trauma. The literature reports hernia as a complication of omental flap reconstruction, but as yet there is little information on the laparoscopic repair of large subxiphoid hernia resulting from omental flap reconstruction. Case: A 52-year-old woman presented 2 years after surg...
متن کاملCombined vacuum-assisted closure treatment with laparoscopic mobilization of an omental flap and meshed skin grafts for reconstruction of infected sternotomy wounds: two cases.
In cardiac surgery, poststernotomy wounds are life threatening complications, with mortality up to 50%. We described two patients, who underwent coronary artery bypass grafting and postoperatively developed a deep sternal wound infection. Reconstruction was combined with vacuum-assisted closure treatment, laparoscopic mobilization of an omental flap and split thickens skin grafts. The omental f...
